What you need when making an appointment
You will need the following information when making an appointment.
- Insurance Name
- Name of primary insurance holder
- Primary holder’s Scocial Security number and DOB
- Name of primary insured’s employer
- Member ID and group # if any
- Name of patient and DOB
***Please bring your insurance card and ID for your appointment.***
