What you need when making an appointment



You will need the following information when making an appointment.

  • Insurance Name
  • Name of primary insurance holder
  • Primary holder’s Social Security number and DOB
  • Name of primary insured’s employer
  • Member ID and group # if any
  • Name of patient and DOB
